Restaurant Website Intake Form

Fill this out once and send it to your website designer. Every menu item is tagged by meal period (lunch, dinner, both, or all day) and includes description, photo, estimated calories, and gluten-free status. Drag the ⋮⋮ handle to reorder items; numbers update automatically.

1. Restaurant identity

Name, contact, and how guests should find you. This becomes the header, footer, and Google listing content.

2. Hours, reservations & service style

Be exact. Designers put this on every page. Note kitchen close times separately from bar close if they differ.

3. Story, chef & atmosphere

This copy feeds the About page, homepage hero, and schema description.

4. Appetizers

Maximum 7 appetizers. Each item must state whether it is served at lunch, dinner, both, or all day. Attach a photo when you have one.

5. Soups & salads

Maximum 7 soups and salads combined. Use the description to mark Soup or Salad. Identify lunch vs dinner availability on every row.

6. Lunch entrees

Maximum 7 lunch entrees. These items are tagged as Lunch by default. Change the meal period only if the same plate also appears at dinner.

7. Dinner entrees

Maximum 7 dinner entrees. These items are tagged as Dinner by default so the designer never mixes lunch and dinner plates.

8. Sides & shareables

Maximum 7 sides or shareable plates.

9. Desserts

Maximum 7 desserts.

10. Drink menu

Maximum 7 drinks. Separate house cocktails, wine, beer, coffee, and non-alcoholic drinks. Calories and gluten-free still apply (beer, malt, some mixers).

11. Wine list

Maximum 7 wines. Use glass and bottle prices. If a wine is poured at both lunch and dinner for different amounts, choose Both lunch and dinner to open those extra price fields.

12. Specialty drinks

Maximum 7 specialty cocktails or signature drinks. Numbered Specialty drink 1, Specialty drink 2, and so on.

13. Happy hour menu

Maximum 7 happy hour items (food or drink). Include the regular price and the happy hour price on each row.

14. Food bar menu

Maximum 7 food-bar or bar-bites items (wings, sliders, nachos, raw bar, late-night plates). Tag lunch, dinner, both, or all day on each item.

Additional photos for the site (hero, dining room, patio, team, plated dishes).

17. Save, local database, and email

Drafts and submissions are stored in a local browser database (IndexedDB). Submit also writes CSV and Excel. If the optional Python server is running, the packet is posted there so it can email kim@adaptivesolutionsonline.com. If the server is offline, the form still opens a client-side email draft.

Run python3 intake-server/server.py to enable server-side storage and SMTP email. Leave the endpoint as-is for local use.
